Forms for Wildcats - PLAYERS
The forms below are the for all players and parents of West Wyalong Wildcats to read and sign. If you have the facilities you can print, read and sign the forms prior to Monday night and bring them along to hand to your Manager. Forms for the players include Code Of Conduct/Player Behaviour Form and Photo and Video Permission.
Uniform order forms are also available below. Samples of shorts and training singlets will be available on Monday evening to check for sizing.
