If you cannot pay your yearly insurance amount on-line this form can be used and must be handed back with payment at the Sports Office. Note the Sports Office can only take cash payments.

Note if not paying on-line you are going to pay approximately going to pay $8 more due to an adminsitation fee for the manual processing of the payment.
