INSURANCE CLAIM FORMS

As of September 1, 2013, Basketball Victoria has changed insurance providers from PSC Horsell to V-Insurance Group (Corporate Authorised Representative of Willis).
For claims relating to injuries that occurred after 4pm on September 1, 2013, a Claim Form can be downloaded by clicking HERE. To download a Summary of Cover, please click HERE. There is also other useful information you may require on the V-Insurance Group you can find by clicking HERE.

For injuries that occurred prior to 4pm on September 1, 2013, please refer to the Claim Form link below.
All claims will need to be sent directly to the Horsell Sydney office at the following address:

NOTE: When submitting a claim form, please note that Section B, the 'Association Declaration Section' must be filled out by your association before sending to Horsell.
If this is not done, the claim will not be processed, as Horsell will not believe the player to be registered for insurance.
CERTIFICATES OF CURRENCY
If you require a Certificate of Currency for insurance purposes please email your request in writing to: admin@bvcc.net.au
Please note that all certificates will have association names on them only. For clubs that require a certificate for their training venues, please ensure you specify which association you are affiliated with, or contact your association directly for a copy.
