For those injured playing basketball in Victoria as registered players of the CYMS Basketball Association, Basketball Victoria has Basketball Victoria has brokered Insurance for all registered players with our Association.

Below are links for current Insurance Information and the Insurance Claim Form which needs to be completed by those wishing to make a claim.

Please read the information and complete the form before submitting to CYMS for completion of the section verifying the injury took place during a CYMS game.

Please note that on Page 6, the 'Association Declaration Section' must be filled out by CYMS before sending the form to the insurer. If this is not done the claim will not be processed as they will not believe the player to be registered for insurance.
