2013 SVNSW Affiliation & Updated Membership Form
It's shaping up as a busy year for SVNSW and we look forward to working with you all again this year.
The Pack contains:
1. 2013 Affiliation & Member Registration Fee Schedule, incorporating a description of the Benefits and Conditions of Affiliation and Membership of State Volleyball NSW;
2. 2013 Affiliation Form for Clubs and Associations wishing to Affiliate in 2013. This must be completed in full and accompany payment of the appropriate Affiliation Fee. There has been no increase in Affiliation Fees for 2013;
3. 2013 Membership Application Form, incorporating the Member Declaration; and
(NB: Membership Application Form updated 25/1/13. SVNSW has rectified an omission in paragraphs 2 and 5 and now increased the space for members to insert their Affiliate Association/Club name.)

4. SVNSW Player Accident Insurance Summary, setting out important information regarding the coverage and benefits of the 2012/13 Insurance Policy, as well as the process for lodging a claim for an injury. You will note the Policy Period and also the Policy Number to assist in your management of Member injuries.
