This form must be prepared in triplicate and each copy signed by the player concerned and by the Secretary of the club granting the permit prior to the player taking the field with the Major Club to which the permit is granted.

ORIGINAL copy signed by the opposing Clubs Secretary or Captain prior to the player taking the field must be forwarded by the Secretary of the Club receiving the permits to the Secretary of this Major League, not later than the first mail following the match for which granted.

DUPLICATE copy must be forwarded by the Secretary of the club granting the permit to the Secretary of his League not later than the first mail following the match for which granted.

TRIPLICATE copy must be retained by the Secretary of the club granting the permit. All copies must be filed and kept available for future reference (if required by the respective Secretaries concerned).
