Notification of Availability

Use this form to provide notification of your availability for appointments or, more importantly, unavailability.

You must submit notice of unavailable (or "closed") dates at least fourteen days in advance of the affected date.

That means that submitting on Monday morning is too late for the weekend which would then be only 12 days ahead!

Notification must be sent to both NSW State League Football Referees, and the Football NSW Appointments Officer. This form will do both.

If you are using an email client such as Outlook or Notes, then you can email directly from the form.

If you are using a web browser then you will need to fill out the form, save a copy of the completed form, and then attach the saved file to a new email message.
