FNSW Updated Insurance Information – June 2014
PERSONAL INJURY INSURANCE DOCUMENTS
The attached documents should be provided to injured participants for claim under the Personal Injury Insurance provided by FNSW.
The Personal Injury Claim Form and the Personal Injury Cover Summary provides information about claimable benefits and How to claim.
Claimants should read through the documents and complete the Claim Form.
It is the responsibility of the claimant to arrange for the claim form to be completed, including the Physicians Statement.
Clubs should ensure they have a record of the injury and the player was registered at the time of the injury before signing the Club Declaration.
COMPLETED CLAIM FORMS
Claim forms must be posted to QBE Claims Department, GPO Box 4108, Sydney NSW 2001 or emailed to accidentandhealth@qbe.com.
The claimant should make a copy of their claim form and keep for their records if posting.
Once a claim has been assessed by QBE they will write directly back to the claimant and advise the outcome of the claim.
Receipts for reimbursement of claimable benefits can be sent to QBE with the Claim Form or at any time after receiving reply from QBE.
INFORMATION ABOUT THE PERSONAL INJURY INSURANCE
The Personal Injury Cover Summary (as attached) provides participants with general information about the benefits claimable.

PUBLIC LIABILITY & CERTIFICATES OF CURRENCY 2014
When hiring grounds and venues, Councils and venue providers will request a copy of a Certificate of Currency for Public Liability purposes.
Certificates of Currency are available for each registered club/association/branch. A copy of the form has been email to clubs - if you require same again please contact Jennifer Popov at admin@footballcanterbury.com.au.
Please NOTE: For risk management and Public liability purposes please ensure that a record of all accidents is recorded by the relevant club/association.
Serious accidents should also be reported to the Football NSW Risk Manager.
New Insurance Website
JLT has developed a new insurance website for FNSW and its membership.
All clubs/associations/branches/referee branches are requested to place the new URL on your website for easy access by your members to insurance information www.jltsport.com.au/FNSW
The new website will go live on 1 January 2014.
Members will also be able to access the new JLT website via the FNSW Insurance link located under Corporate in the footer menu of the FNSW website at http://www.footballnsw.com.au/index.php?id=524.
If you have any queries regarding the insurance please refer to the new JLT website at the first instance or contact your Association, Branch or Football NSW.
(Please note that the FNSW Insurance webpage is currently being updated with the new documents and information so please allow time for this to be completed).
