Request for Game Change Form
For your convenience of Club Competition Secretary's, and also the Fixtures Committee, a form has been devised for you to load up and fill out in case of the need to move games.
Please note on the form, there are “fill in” spots where you can add information electronically before printing for signature.
All forms must be sent to the Fixtures Committee at least twenty one (21) days prior to the scheduled game on the draw, which is required to be moved.
All forms should be forwarded to Trent Thomas either via email at trent@footballcanterbury.com.au or via fax to 9716-8559.
Please note that both Clubs must sign off on this request before forwarding to the Fixtures Committee for approval.
