How to register and what to bring

Please read carefully as all requirements must be satisfied.

1. Form & Documents, All players Under 10 to Under 16. New and Old players.

• Fill out WJFC Registration Form
• Fill out DRJFL Registration Form
• Fill out WJFC Medical Information Form
• Fill out Privacy Permission Form
• Provide proof of Date of Birth eg. Passport, Birth Certificate, Baptismal Certificate, Health Records Immunization Record
• All Forms available on this website or on Registration Day February 8th 2009

• 2009 Fees per child $80.00 with each additional sibling $10.00 Payment can be made either by cash or cheque to:
